Diabetes Patient Registry Pilot Project

New York Diabetes Coalition project to improve primary care practices’ diabetes care through implementing Web-based diabetes registries. Funded by the New York State Department of Health, this program provides interactive education, implementation and data assistance, and 18 months of support for licenses to a Web-based registry, DocSite.
