The Migrant Clinician Network (MCN) Diabetes Track II project is designed to track mobile individuals with diabetes. Track II Program thereby encourages continuity of care with the transfer of vital health information between clinics, assistance to finding and accessing services, and support as they implement their treatment plan.

Active Living in Santa Ana is a local YMCA-led partnership promoting physical activity in Santa Ana, California, using a community-organizing approach and focusing on low-income public housing developments, schools, and neighborhoods. Includes components such as developing and maintaining walking and exercise groups at public housing facilities and working with neighborhood associations to identify recreational opportunities and barriers to active living.

American Pharmacy Association Foundation reports on the Asheville Project, an employer-sponsored pharmacist-coaching program to improve outcomes and reduce care costs for patients with diabetes and other diseases.
