The New York State Health Foundation’s Diabetes Policy Center congratulates Monroe Plan for Medical Care for becoming the first plan to successfully complete the Diabetes Incentive Program Endorsement.
The New York State Health Foundation’s Diabetes Policy Center congratulates Monroe Plan for Medical Care for becoming the first plan to successfully complete the Diabetes Incentive Program Endorsement, developed by the Diabetes Policy Center.
Monroe Plan’s implementation of an effective diabetes incentive and reward program demonstrates their commitment to members with diabetes and the providers who care for them.
Monroe Plan has shown tremendous leadership in improving outcomes for their members with diabetes. The plan supports its members’ successful management of diabetes through multiple methods, including reaching out directly to members to identify gaps in diabetes care, making referrals to certified diabetes educators for self-management support and diabetes support groups, and sending reminder notices for routine and follow up care—all necessary for successful management of diabetes.
In addition, Monroe Plan supports providers by offering diabetes care management fees, providing patient outcome reports to providers, and financially supporting care managers at the practice sites in order to provide team-based care. In 2008, with a grant from the Center for Health Care Strategies, funded by the Robert Wood Johnson Foundation, Monroe Plan assisted 37 physicians in achieving recognition under the National Committee for Quality Assurance’s Diabetes Recognition Program.
