Primary care physicians are one of the most important sources of ongoing treatment, management, and information for patients with diabetes. Primary care physicians are trained to provide continuous and comprehensive care and can help patients manage their diabetes over the long-term and avoid complications. The Campaign and its partners will work with primary care physicians across the State to provide them with the tools and support they need to improve diabetes care and outcomes. Specifically, the Campaign will help physicians and practices achieve these outcomes and processes for their patients with diabetes:
