Health Care Providers

Although there is ample information available on best practices in diabetes care, many patients still do not receive the care and support they need to manage their diabetes effectively. In response, the Campaign will work with a network of partners to measurably improve the care and outcomes for patients with diabetes. If you are a member of one of the Campaign’s clinical partners (listed below), please contact them to assist you in improving your diabetes care.

The Campaign is currently accepting request for proposals (RFP) to help primary care physicians achieve national recognition for excellent diabetes care. The Meeting the Mark: Achieving Excellence in Diabetes Care RFP invites providers, health organizations, insurers, and other stakeholders throughout New York State to apply for funds to improve diabetes quality of care and to support health care providers in achieving recognition by the NCQA or BTE diabetes programs.
