730,000 New Yorkers don’t know they have diabetes.
Health Care Providers
Although there is ample information available on best practices in diabetes care, many patients still do not receive the care and support they need to manage their diabetes effectively. In response, the Campaign will work with a network of partners to measurably improve the care and outcomes for patients with diabetes. If you are a member of one of the Campaign’s clinical partners (listed below), please contact them to assist you in improving your diabetes care.
- The Community Health Care Association of New York State
- The Healthcare Association of New York State
- The New York Chapter of the American College of Physicians
- The New York State Academy of Family Physicians
